Community Support Services, a Division of Gaston Skills, Inc., believes in incorporating the highest ethical and legal standards in providing services to our consumers. We also strive to preserve the dignity of persons served and their families within their community. Community Support Services is committed to ensure that all personal information is secured in a safe and confidential environment while receiving services at our facility.
What is Community Support Services?
Community Support Services is a Division of Gaston Skills, Inc. that has been providing services to adults in Gaston, Lincoln and Cleveland Counties for 5 years. Community Support Services has a staff of 22 Qualified, Associate, and Paraprofessionals who are dedicated to enhancing the lives of persons with the following diagnoses:
- Mental Health
- Mental Health with Substance Abuse
- Developmental Disabilities
The services we provide are:
- Community Support Adult
- Community Support Team
- Community Support Group
- Targeted Case Management
Community Support Services will provide the coordination of community services and support to meet an individual's physical, mental, emotional, social, vocational and community integration needs. Our services enable the person served to live in the least restrictive environment possible and increase their adaptive capabilities.
Community Support Services Qualified Professionals provide persons served with a highly individualized and integrated treatment plan. The treatment plan extends the process to include the family system, healthcare coverage and the community.
Community Support Services Qualified Professionals are familiar with community resources so that they can provide for the persons served the following needs:
- Physical
- Substance Abuse with Mental Health
- Mental
- Emotional/ Behavioral
- Social Integration
- Vocational Opportunities
- Community Integration/ Community Resources
- Targeted Case Management for persons with Developmental Disabilities who require specialized services

A Qualified Professional will assist you in the following services:

- Employment Opportunities
- Applying for Medicaid or Disability Benefits
- Links to Transportation
- Links to Community Based Services and Resources
- Scheduling Doctor Appointments (Counseling, Rehabilitation, etc.)
- Developing Recovery Skills
- Developing Job Skills
Admission Criteria:
1. Age range of persons served must be adult consumers 21 yrs of age and older.
2. Have an Axis I (mental illness) or Axis II (developmental disability) diagnosis.
3. The person served must be in one of the Disability Groups:
- Mental Health
- Mental Health with Substance Abuse
- Developmental Disabilities
- Live in Gaston, Lincoln or Cleveland County
The person served must have at least (2) identified needs in one or more of the following areas:
- Financial
- Medical/ Medication
- Social
- Residential
- Guardianship/Power of Attorney
- Advanced Directive
- Psychological/Behavioral
- Vocational/ Educational
Referral Process:
All referrals to Community Support Services must first be screened through the Pathways Screening, Triage, and Referral (STR) telephone line: 1-800-898-5898.
A clinician from Pathways will determine if the person served needs enhanced benefit services (Community Support Services/ Targeted Case Management) or basic benefit services (Medication Clinic/ Therapy). If the clinician determines that the individual needs enhanced benefit services, then the person served will be given a choice of which agency he or she wants to provide the services. If the individual chooses Community Support Services, a Division of Gaston Skills Inc. then the clinician will notify our agency.
